After an accident I was involved in earlier in the year, the DFSOR report has now been completed and closed. After going out of my way and ringing them up, the MAA have emailed me a copy of the report which to be honest isn't as in depth as I would have hoped.

However, there is a word doc link on the report which i am not auth'd to open as it is marked 'Management in Confidence'. My boss can read it but he can't let me and also can't tell me what details it contains. I'm finding this really frustrating as although I have not been found at fault for the accident, I would like to know what they have written about me and as I am the subject of the report, surely by law, i should be able to read it?

Submit a Freedom of Information question to the Government, they will then have to forward it to the MoD and by law they must answer.

It is absolutely and utterly against all "Error Managment Systems" Policies to keep the information found in error investigations secret - and it shows the system is completely "IMMATURE" if not infantile in its pretended aims.

FOI should be used to open it up in ALL Reports until they (ASIMS? MAA?)realise that is is more expensive to keep something unnecessarily "secret" when the whole point is to have it OPEN and HONEST.

If it's Names then they are reporting incorrectly. If it's Causes then they are reporting inaccurately.

Either way points to incompetence in running the system.

ASIMS is open and unclass, phone the helpdesk they will be able to give you access to the whole report.
No names should be present and where errors have been made by an individual or organisation there should be no blame. Its about learning and changing. The system will work if trusted and used properly, some people still need to learn this. I can help if you need it.
