Depends on local arrangements, so places have a bespoke form, others use a gen app.

Not as far as I'm aware. However if you couldn't drive and I were your Flt Cdr I would ask how you intended to get to and fro and what arrangements you had in place for emergency call outs etc.

The app is usually a locally produced form, which you should be able to get from your admin office.
