Quick question, do you have to give a written mid point or can it just be a verbal? I have seen a form - if I use this does it have to be completed comprehensively or will bullet point format do?

It can be verbal but the date of de-brief should be recorded somewhere. Depends if you think the 1RO/Subject will have any issues, if so better to have it written so there is no ambiguity as to what was agreed come the end of reporting period.
 
I believe that it should be written with a copy given to the subject and a copy retained for your records and then destroyed when you write the annual SJAR. It is laid out in the JSP I think it is 752. Hope this helps.

JSP 757, I always do a written MPAR which is destroyed when I do the SJAR, it gives you and the subject something to work from and is evidence of strengths/weaknesses etc, which you won't have with a verbal MPAR. The date you do the MPAR is recorded on the SJAR.
